Completed forms may be submitted by email or fax; however, they must include a wet (ink) signature to be processed. Electronic or digital signatures are not accepted at this time.

As a retired member of the NCBAA Trust Fund, you will be able to access your benefits by submitting claims for medical expenses incurred.
You will be able to submit health claims electronically by logging into Sun Life; however, some providers have the option to direct bill.
